Cedar Hill Pain and Rehab began in 2007 with one doctor and no employees. Today, we are in a 5,000 square foot facility with 4 physicians, 3 nurse practitioners and a procedure center.
Our patients are important and their quality of care and treatment outcomes are extremely important. We treat the whole patient and design our care around first addressing why the patient has pain, and then developing a plan to treat the source.

Please bring current drivers license, insurance card, and recent imaging (MRI, X-ray). In addition, please bring the paperwork which was emailed to you fully completed. You can also get this paperwork by clicking here
We take most major insurance carriers, including some work comp plans (Dept. of Labor). We do not take any form of Medicaid. If you would like to see if your insurance is accepted, please consult our website or call the office at 972-291-9165 and choose option 4.

Please arrive 15 minutes prior to your first appointment to allow for time necessary to complete your new patient paperwork. Please bring your insurance card and photo identification. It is strongly recommended that you print new patient paperwork at home and complete prior to initial consultation.
